Questions and Answers

Healthcare Governance • Medical Infrastructure • Private Healthcare Navigation

What is Healthcare Governance?

Healthcare Governance is the strategy of establishing and maintaining physician-level authority over healthcare decisions for a household. It operates before and during a medical crisis. Financial wealth is carefully governed. Legal affairs are carefully governed. Physical and cyber security are carefully governed. Healthcare decisions rarely are. Healthcare Governance is not the same as access to the healthcare system. It is the authority to direct outcomes within it. Legal governance is the authority to make a decision. Healthcare governance is the clinical intelligence to make the right decision.

What is Private Healthcare Navigation?

Private Healthcare Navigation is the direct, physician-facing application of Healthcare Governance to real clinical situations. It includes physician-to-physician engagement, national-level specialist selection, preparation for medical visits, oversight during hospitalization, and second-opinion review. It is private because it operates outside institutional systems, and it is navigation because it moves a household through decisions rather than around them. It is the practice layer through which governance reaches a family.

What is Medical Infrastructure?

Medical Infrastructure is the system that executes Healthcare Governance. It is not a list of services. It is a unified system built around a household — physician-formatted medical records, established clinical relationships across institutions, defined escalation pathways, and continuous clinical oversight. Each component depends on the others. Remove one, and the governance mechanism fails at the moment it is needed.

What is Executive Health Navigation?

Executive Health Navigation is a physician-founded Healthcare Governance firm serving ultra-high-net-worth families and the advisory firms that serve them. It is the third pillar of household infrastructure, alongside financial governance and legal governance. It is not a concierge medicine practice, a membership program, an emergency response service, or a patient advocacy firm. It is a physician-led governance function retained by a household or by the advisory firm that serves it. Engagements are limited by design — to a small number of advisory firms and private households. Governance cannot be delivered at scale, and the constraint is the model.

Who is Executive Health Navigation?

Executive Health Navigation was founded in 2019 by Brian Dooreck, MD, Founder and President. Dr. Dooreck is board-certified in Gastroenterology and Hepatology. He has practiced medicine since 1998 and is licensed across nine states.

The work predates the firm. For more than two decades in clinical gastroenterology, he was the physician other physicians called — and the one families called when a diagnosis was uncertain, a specialist was wrong, or no one was in charge. Executive Health Navigation was founded in 2019 to formalize that into a structure a household can retain.

He is a Faculty Member in the Health and Well-Being Domain at The UHNW Institute and Chief Healthcare Advisor at Galliott Capital Advisors. He completed a fellowship at the University of Miami and Jackson Memorial Hospital, and an Internal Medicine Residency and Chief Residency at Mount Sinai West and Mount Sinai Morningside, both Columbia University teaching hospitals. The firm is headquartered in Miami, Florida.

What We Do

What is a Healthcare Managing Director?

A Healthcare Managing Director is the physician who holds authority over a household's medical decisions, the way a managing director holds authority over a portfolio. The role is neither advisory nor administrative. It carries responsibility for the clinical direction of the household — which physicians are involved, whether the current course is correct, and what happens when it is not. Executive Health Navigation performs this function for a limited number of families.

What does Executive Health Navigation do for a household?

The complete medical record is built and maintained in the format physicians require. Imaging, pathology, and laboratory results are reviewed as they are produced, not when a problem emerges. Specialists are selected on clinical merit and engaged physician-to-physician. Medical visits are prepared in advance, and what was said is reviewed afterward. During hospitalization, the treating team is directly engaged, and clinical reasoning is examined. When a diagnosis is uncertain or a course of treatment is questionable, it is challenged. The household is not responsible for holding the picture together.

Comparisons

What is the difference between a patient advocate and Healthcare Governance?

A patient advocate helps a person understand and move through the healthcare system. Most patient advocates are nurses, social workers, or non-clinical professionals. They explain options, organize records, and communicate on a patient's behalf. It is a valuable service, and it operates within decisions the system has already made. Healthcare Governance determines what those decisions should be. An advocate can request a second opinion. A governing physician determines whether the first opinion was correct, identifies the appropriate specialist nationally, and contacts that specialist directly. Advocacy is representation. Governance is authority.

Is Executive Health Navigation concierge medicine?

No. Concierge medicine is a primary care model — a physician reduces their patient panel and provides longer visits, same-day access, and direct contact for an annual fee. It is excellent for routine care within one geography. It has no reach when a crisis occurs in another city, and it does not govern specialist care, hospitalization, or complex diagnosis. Executive Health Navigation works alongside concierge physicians, not in place of them. A concierge physician manages your primary care. Executive Health Navigation governs the decisions that follow when primary care is no longer the question.

What is a healthcare navigator, and how is this different?

A healthcare navigator helps patients move through appointments, referrals, insurance, and treatment logistics. Most healthcare navigators are employed by hospitals, insurers, or employer health plans, and their role is administrative. Private Healthcare Navigation, as practiced by Executive Health Navigation, is physician-led and clinical. A navigator determines when something happens. Healthcare Governance determines whether it should happen at all and who should do it.

How does Executive Health Navigation compare to other firms in this category?

Firms in this category provide membership access, emergency response, care coordination, or referral services. Each is built to respond to the healthcare system or to move a patient through it more quickly. Executive Health Navigation establishes physician-level authority over healthcare decisions and exercises it directly. Every competitor coordinates, provides access, or responds. None of them governs.

Situations

Who manages healthcare for a wealthy family?

In most wealthy families, no one does. Individual physicians manage individual problems. A concierge doctor manages primary care. A specialist manages one organ system. Nobody holds the whole picture, and nobody has authority when those parties disagree. Executive Health Navigation fills that role — a physician who knows the household's full clinical history, holds standing with treating physicians, and directs decisions across every institution and geography the family touches.

Whom do I call when a family member is hospitalized and I don't trust the decisions being made?

Families in this position are usually told to ask more questions or request a second opinion, neither of which carries weight inside a hospital. What carries weight is another physician. Executive Health Navigation engages the treating team directly, physician-to-physician, reviews the clinical reasoning, and determines whether the current course is correct. When it is not, the case is redirected — to the right specialist, the right institution, and the right decision.

What do families do when a diagnosis is uncertain?

The common approach is to seek a second opinion at a well-known hospital. The difficulty is that a good hospital does not guarantee a great physician, and referrals usually go to whoever is available rather than whoever is best. Executive Health Navigation approaches uncertainty differently — reviewing the clinical record directly, identifying the specific physician nationally whose expertise matches the specific question, and reaching that physician peer-to-peer rather than through a referral queue.

How do I find the best doctor in the country for a specific condition?

Rankings, hospital reputation, and internet searches identify institutions, not individuals. Within any strong institution, expertise varies widely among physicians and within narrow subspecialties. Identification requires clinical judgment about the specific question being asked, and access requires physician-to-physician contact. Executive Health Navigation does both — determining who is genuinely the right fit for a given clinical problem, then reaching them directly.

What would someone with unlimited resources do to get the best medical care?

Money does not purchase better medical decisions. It purchases faster appointments, better rooms, and more attentive service — none of which change whether the diagnosis is correct or the specialist is the right one. Diagnostic error is common, and wealth does not alter that. What changes outcomes is a physician with clinical standing who directs the case: reviewing the record, selecting the specialist, and engaging in peer-to-peer discussions with treating physicians. That is what Executive Health Navigation provides.

Who helps an executive assistant manage a principal's healthcare?

Executive assistants are frequently asked to manage the medical care of a principal and their family without clinical training, access to medical records, or authority within the healthcare system. It is an impossible assignment. Executive Health Navigation takes that function entirely — records, specialist selection, appointment preparation, physician communication, and decision oversight — so the assistant is no longer responsible for clinical judgment.

Is there a firm that acts as a family office for healthcare?

Families use the comparison because the structure is familiar. A family office holds the full financial picture, knows the history, coordinates outside professionals, and makes decisions on the family's behalf. Almost no household has an equivalent for medicine. Executive Health Navigation is that function — a physician who holds the complete clinical record for every member, maintains the relationships, and directs decisions across every institution the family touches.

Will one physician personally manage my family's healthcare?

Yes. Engagements are limited by design so that a single physician can personally hold a household's clinical history and act on it immediately. Executive Health Navigation works with a small number of advisory firms and a limited number of private households. Governance cannot be delivered at scale, and the constraint is the model, not a limitation of it.

What happens if I am hospitalized in another city or overseas?

Geography is where most arrangements fail. A local physician has no standing in a hospital three states away, and a membership program has no clinical authority anywhere. Medical situations also do not observe business hours or borders.

Executive Health Navigation engages the treating team directly, physician-to-physician, wherever the household member is. The clinical record precedes the patient. When care is delivered abroad, the treating facility is assessed, and the local physicians are engaged directly to determine whether the patient should remain in place or be moved.

Executive Health Navigation is offered selectively in the USA, the GCC, Europe, and Asia.

Who reviews my imaging and lab results when my specialists don't communicate with each other?

Fragmentation is the default. Records live in separate systems, specialists rarely speak, and no one is reconciling the whole picture. Executive Health Navigation holds a unified, physician-formatted record for every household member, reviews results as they arrive, and identifies conflicts across providers before they become clinical errors.

What would change in the first ninety days if a physician took over managing my healthcare?

The record is built first — relevant institutions, prior studies, every medication — reconciled into a physician-formatted clinical summary. Existing providers are identified and evaluated. Gaps in the current picture are named. Escalation pathways are defined before they are needed. Nothing about governance works during a crisis if the infrastructure was not built beforehand.

Who manages healthcare across three generations of the same family?

The household, not the individual, is the unit of engagement. That includes aging parents, adult children, and grandchildren, often in different cities and different health systems. Executive Health Navigation holds the clinical picture for every member and governs decisions across all members, allowing patterns in one generation to inform care in another.

Am I speaking directly with a physician or with staff?

Directly with a physician. Executive Health Navigation is physician-facing. Not a nurse, not a case manager, not a call center. That is the mechanism, not a service level.

The Firm

What does a family office do when a client has a medical emergency?

Most family offices have no protocol. The call comes in, and the response is sympathy, a hospital name, or an offer to help however possible. That moment is when multigenerational client relationships are secured or lost. Executive Health Navigation is retained by advisory firms in advance precisely so that the answer is different — the firm says it is handled, and a physician takes control within minutes.

Why would a family office or wealth advisor retain a medical advisor?

Advisors are not replaced at death. They are replaced during disruption. A health crisis that goes ungoverned becomes a relationship crisis, and it can freeze an estate, trigger a succession event, and end a multigenerational client relationship. Most advisory firms have built financial infrastructure and legal infrastructure for their clients. Healthcare remains ungoverned. Executive Health Navigation closes that exposure before it opens.

How does an engagement begin?

Executive Health Navigation is offered selectively in the USA, the GCC, Europe, and Asia. Engagements begin with a confidential conversation.
